Obtaining BES Client Access Licenses (CAL) for a handset:
As an alternative to buying bulk license packs for BES server, each Blackberry handset comes with 1 BES Client Access License.  You can obtain the license key by registering your handset at the following address:

Go to http://www.blackberry.com/select/professional/express.shtml
Blackberry remote provisioning (Server)
1) On the BES server, Add a user
2) Right click on the user and "Set Activation Password"
  -or- Right click and "Generate and Email Activation Password"
First you need to WIPE the phone:
Wiping your Blackberry Handset (factory reset)
1) Go to Settings / Options
2) Select "Security Options"
3) Select "General Settings"
4) Click the menu key
5) Select "Wipe handheld"
6) Click Continue
7) Type in the word "blackberry"
Then you need to remote provision your phone:
Blackberry remote provisioning (Blackberry User)
1) Power on your phone and enable the Wireless (so it's ready to make calls)
2) Go to the Settings > Time & Date and change the time zone to NZ or Australia and make sure the time is right.
3) Go back to Advanced Settings > Enterprise Activation
4) Enter your email address, and the activation password provided
5) Press the menu button, and choose "Activate Now". then wait, and your phone will display the following (or similar):

Activating user@domain.co.nz... (if it hangs here for more than 1 minute, press the menu button and choose "Cancel", then repeat from step 3 above and it will work troubleshooting)
Verifying encryption...
Encryption verified.
Waiting for services... 
(can hang here, see troubleshooting)
Services received.
You email address,
user@domain.co.nz is now
enabled.

Activation hangs when "Activating user@domain.co.nz..."
Activation may hang at this point, if it hangs for more than 1 minute, press the menu button and choose Cancel. Repeat from 3) Go back to Advanced Settings > Enterprise Activation and the activation process will complete.

Activation hangs when "Waiting for services..."
Remove the battery for about 30 seconds and repeat the provisioning process.  Also notify your administrator so they can "Resend Service Books" to your phone.

I am missing my contacts? 
Contacts are the last thing to Synchronise, and can take up to 24 hours if you have a lot of contacts.  If you still have no contacts after 24 hours, please try restarting your phone by removing the battery.  Once turned on, wait another 15 minutes - and if still no contacts have appeared, please contact your BES administrator.

Licensing
There are currently 2 flavours of Blackberry server available:

Blackberry Express (30 users maximum - before upgrading to Enterprise)
Blackberry Enterprise Server (14 users max before full licenses required)

There is currently no migration path from BES to Express or vice versa.
